How to convert a form from a form template and create a new form

Forms can directly be created from a template using following steps:

1. Selects form template from the left hand menu.
2. Hover over the desired template and click the duplicate button.
3. Fill in the form name and description in the dialogue box and click ‘Duplicate Form’ button.
4. New form is created and form is opened in edit mode so that modifications can be done if needed otherwise form is ready to use.
